A Clinical Review of the 188 Cases of Peptic Ulcer Perforations



Abstract
This report is a clinical review of 188 cases of peptic ulcer perforations that had been treated for 5 years from Jan. 1st 1986 to Dec. 31st 1990 at the department of surgery in Dae Dong Hospital.
@ES The results were as follows.
@EN 1) The male to femal sex ratio was 10 : 1 and the mean age was 37.9 year-old.
2) The chief complaints were abdominal pain(92.5%), nausea and vomiting(26.6%) and hunger pain in desending orders. Adominal tenderness and rebound tenserness showed 69.4% in cases and abdominal tenderness, rebound tenderness and board-like
rididity
showed 36% in cases simutaneously. The main sites of absdominal pain were epigastric (46.8%) and diffuse(38.3%).
3) Anemia(4.9%), hemoconcentration(36.2%), WBC counts under 10000/§§(33.5%) and electrolyte imbalance(16.0%) were observed in their hematologic findings of cases.
4) The 86.3% of cases received the operation under 24 hours after the onset of perforation symptoms. The 93.1% of cases received the definite operative procedures and the 75% of cases did the vagotomy and pylorolasty.
5) The complications occured in 25% of cases. The 9.0% of cases were proceure-specific and 16.5% of cases were procedure nonspecific. The differences of complication rates according to every 6 hours interval of time lag did not show the
satistical
significance.
6) The accompanied diseases were observed in 20.2% of cases and were pulmonary tuberculosis, hypertension and diabetes mellitus in descending orders.
7) The mortality rate was 2.1% and the cause of death was not related to sepsis but related to the advanced age and the accompanied disease in this study.
8) The drainage was no more benifit in postoperative care and induced the longer hospitalization significantly in the cases of 5 days or more usage.
9) The hospitalization period, complication rate and mortality according to 24 hours interval of time lag did not reveal the statistical difference.
